Person-Centered Assessment and Planning

The intent of this training is to describe and show examples of the process of assessment, interpretive summary, and ultimately the development of the person-centered plan. This training, provided from a Behavioral Health perspective, will provide an actionable combination of interviewing protocols and relational techniques to assist in this process.

Learning Objectives: 

  • Understand the importance of a fluid person-centered assessment and plan.
  • Know how to take the interpretive summary and translate it to the person-centered plan.
  • Create a person-centered plan that meets CARF standards.
  • Understand assessment of progress and treatment plan updates, transitions, and discharges.

Presenter Information:

With over 26 years of experience as a licensed professional counselor, Bonni Stephenson is a skilled mediator and uses intuition and experience to know what is needed in the moment. She currently works both as a consultant for CARF accreditation and clinical liaison and supervisor for disruptive event/trauma therapists who are placed on military installations worldwide. Prior to working with the military, Bonni spent 16 years with a large CARF-accredited nonprofit behavioral health organization where she was the vice president of outpatient services serving over 5,000 members on an annual basis. Bonni has been a CARF surveyor for the past 21 years and enjoys the rich exchanges and opportunities of being both a collaborator with colleagues and a consultant to organizations.
